Aftercare Services
Recovery continues after leaving the centre. Our aftercare program ensures long-term sobriety and reduces relapse risk. Aftercare provides structure, accountability, and professional support during the transition back into independent living.
Who it is for:
- Individuals who have completed inpatient or outpatient treatment and need continued support.
- Individuals who want to maintain long-term sobriety and prevent relapse.
- Individuals who need guidance adjusting back to daily life, relationships, or work.
- Individuals who want to benefit from ongoing counselling, mentorship, and support groups.
- Individuals who require structured follow-up and access to professionals during recovery.
Services include
- Continued Counselling and Therapy - Ongoing therapy sessions, either in-person or via telehealth, help clients manage new challenges and reinforce coping skills.
- Relapse Prevention Plans - Personalised plans that outline triggers, early warning signs, coping strategies, emergency contacts, and steps to take if cravings or setbacks occur.
- Support Groups and Mentorship - Peer-led groups provide encouragement and accountability. Alumni mentors guide individuals through real-world challenges.
- Access to Professionals - Patients can reach out to our team by phone or telehealth for urgent support, preventing small setbacks from becoming crises.
- Guidance on Relationships and Environment- Focus on rebuilding family connections, workplace reintegration, and managing social pressures. Family sessions and vocational guidance may be included.
- Life Skills Training - Practical support in budgeting, time management, parenting, and job readiness to strengthen independence and reduce relapse risks.
